This useful instrument provides both Self-Report and Observer Report Forms, permitting multimodal assessment of adults with attention problems. Both forms are available in long, short, and screening versions, all measuring a range of behaviors, including:
- Inattention/Memory Problems
- Hyperactivity/Restlessness
- Impulsivity/Emotional Lability
- Problems With Self-Concept
The long forms are composed of 66 items on nine empirically derived scales. Also included are three DSM-IV ADHD Symptom Subscales and an ADHD Index that identifies respondents who might benefit from a more detailed clinical assessment. In addition, an Inconsistency Index alerts you to random or careless responding.
The short forms offer abbreviated versions of the long-form scales, plus the ADHD and Inconsistency Indexes. The screening forms include only the ADHD Index and the DSM-IV ADHD Symptom Subscales (18 items). These versions are ideal when time is limited.
